The Community Team Learning Disability Service provides specialist assessment, support, advice and therapeutic intervention to adults living in West Wales who have a learning disability. These are most often people who have profound and multiple learning disabilities, often with challenging behaviour, or with mental health issues, or additional physical health needs. 

The teams consist of learning disability nurses, occupational therapist, physiotherapist, speech and language therapist, psychologist, psychiatrist, behaviour practitioners and social workers. 

Each team is co-located with social care in bases located in Penlan โ€“ Carmarthen, Ty Elwyn โ€“ Llanelli, Llys Steffan โ€“ Ceredigion, Llanion House โ€“ Pembroke Dock โ€“ Pembrokeshire. The teamโ€™s outreach into the community supporting Primary Care/GPs/Private Providers including clients in supported living and residential units whilst also supporting individuals who are living on their own. The teams also support carers, families, and day services.

How to refer

If you have a learning disability, you can self-refer to the CTLD.  The service will also accept referrals from parents and carers and health professionals.
