Deciding to care for someone who is coming out of hospital and who can no longer care for themselves in the same way as before can be difficult.   The following information is provided to unpaid Carers when the person they will be caring for is in hospital and is being prepared for discharge. It contains information to help the Carer access further support.  

The information you receive can vary depending on whether you are accessing services in Carmarthenshire, Ceredigion, or Pembrokeshire.
